For patients managing multiple conditions, or a high-risk disease, or adjusting to a new diagnosis, this can be a real challenge. 

And we don’t think people should tackle this alone. 

That is why we have a team of pharmacists who work side-by-side with our primary care providers, nurse care managers, and care coordinators to make sure patients get the prescriptions they need. This includes explaining how and when to take medications. And a helping hand to find affordable, cost-effective alternatives when needed.
